We reimagine care delivery with a nurse practitioner (NP)-led model and dedicated care navigators, offering remote-first care and in-person services where needed. Our integrated approach supports both patients and caregivers throughout their cognitive health journey.
APRN Role Overview
We’re seeking a mission-driven Advanced Practice Registered Nurse (APRN) passionate about cognitive health to join our clinical team. The ideal candidate is an experienced, emotionally intelligent clinician who thrives in dynamic, patient-centric environments.
Key Responsibilities:
Direct Patient Care

Deliver empathetic, high-quality cognitive care in a hybrid setting (telehealth and in-person).

Conduct comprehensive cognitive evaluations (history, assessments, labs, imaging).

Develop and manage individualized care plans, including medication management.

Educate and guide patients and caregivers through psychological and behavioral strategies.

Collaborate with primary care providers and specialists to ensure continuity of care.
Clinical Operations & Quality

Contribute feedback to engineering, product, and operations to improve systems and care.

Collaborate with neurologists and clinicians on protocols, care pathways, and data quality.

Engage in case discussions and continuous learning within the clinical team.
Qualifications:
Education & Licensure
-
MSN or DNP required.
-
Certified AGNP or FNP.
-
Active, unrestricted license in Virginia; DC and Maryland licenses a plus (or willingness to obtain).
Experience (2+ years)
-
Caring for patients with MCI or dementia.

Conducting cognitive assessments and neuro exams.
-
Creating personalized, evidence-based care plans.
-
Telehealth experience strongly preferred.

Experience in startups or innovative care settings a plus.
